Neurosurgery, in other words , surgery of the brain and nerves is a medical speciality concerned with the diagnosis and treatment of patients with injury to or diseases/disorders of the brain, spinal cord, spinal column, and peripheral nerves within all parts of the body.
As neurosurgical specialists the neurosurgeons provide surgical and non-surgical care for multitude of conditions including ;
⁃treatment of tumors resulting from brain and spinal cord tissues and/or causing compression to these structures ,
⁃slipped/herniated discs of lumbar and cervical regions ,
⁃injuries to the head and spinal cord ,
⁃diseases affecting brain vessels such as narrowing (stenosis), clot formation (thrombosis), artery blockage (embolism) , rupture (brain hemorrage), aneurysm ( a bulge/ballooning in a vessel ),
⁃vessel narrowings in the neck area ,
⁃neurological developmental disorders of newborn babies ,
⁃epilepsy not responding to medical treatment ,
⁃selected patients with Parkinson’s disease/gait disturbances ,
⁃PAIN MANAGEMENT :  treatment of chronic pain syndromes has an important place in neurosurgery. Science of pain or Algology is a medical discipline concerned with the causes of pain and its origins. It is primarily used to detect the etiology of chronic pain and deals with pain syndromes exceeding 3 months  and their diagnoses and treatments.
⁃Algology aims at diagnosing the cause of pain and implementing the  therapy processes  in patients suffering from chronic pain .
⁃Applications for pain management include (but not limited to) epidural injections , and selective nerve root injections in situations originating from vertebral column .
⁃Procedures employed in treatment of headaches are often used .
⁃Applied with local anesthesia in the neck and upper /low back regions, epidural injections are extremely reliable and among the most commonly performed procedures in patients with spinal stenoses and herniated discs.

ESWT Treatment (Extracorporeal Shock Wave Therapy )

Extracorporeal Shock Wave Therapy (ESWT) is a non-invasive treatment that has been explored as a therapy for erectile dysfunction (ED). It involves using low-intensity shock waves to improve blood flow to the penis, which is crucial for achieving and maintaining an erection. This method is particularly considered for patients with vascular-related ED, where the dysfunction is primarily due to poor blood circulation.

 

How ESWT Works:

1. Shock Wave Application:** During the procedure, a handheld device is used to deliver low-intensity shock waves to various areas of the penis. The shock waves are generated outside the body (hence “extracorporeal”) and are focused on the treatment area.

2. Tissue Regeneration:** The shock waves are believed to stimulate the growth of new blood vessels (angiogenesis) and improve blood flow by enhancing the function of existing vessels. This process is thought to help repair and regenerate the tissues involved in erectile function.

3. Treatment Sessions:** ESWT for ED typically involves multiple sessions, often ranging from 3-6 to 12 treatments over a few weeks. Each session lasts about 15-20 minutes.

 

Benefits of ESWT for ED:

– Non-Invasive:ESWT is non-surgical, requires no incisions, and is performed without anesthesia, making it a low-risk procedure.

– No Downtime:Patients can resume their normal activities immediately after each session.

-Improved Blood Flow:By promoting the growth of new blood vessels and improving circulation, ESWT addresses one of the common underlying causes of ED.

 

Indications:

ESWT is primarily considered for men with:

– Vascular ED:This includes men whose ED is related to conditions like diabetes, high blood pressure, heart disease, or other issues that affect blood flow to the penis.

– Men Unresponsive to Medications:It may be an option for men who do not respond well to oral ED medications like PDE5 inhibitors (e.g., Viagra, Cialis).

ESWT is a promising treatment option for erectile dysfunction, especially for men with vascular-related ED. It offers a non-invasive approach with the potential for long-term benefits, though it is important for patients to discuss the treatment with a healthcare provider to determine if it is suitable for their specific condition. Further research and clinical trials are ongoing to better understand the efficacy and long-term outcomes of ESWT for ED.
